Your name, email and message when you use our contact form, and the details you enter when you book online through Calendly. Please do not send health information through the contact form; we collect your dental history securely at registration.
How we use it
To answer your questions, book appointments, provide dental care and bill your insurance or CDCP. We never sell your information.
